Steps
Confirm NYC Building Code requires the device be inspected/tested twice annually: an annual visual Periodic Inspection plus Category testing.
Distinguish Periodic Inspection (annual visual, Jan 1–Dec 31, filed within 14 days) from Category One (CAT1, annual no-load safety test, filed within 21 days) and Category Five (CAT5, 5-year full-load/full-speed test, filed within 21 days).
File the ELV3/ELV3A report through the DOB NOW: Safety portal using an inspection/testing agency independent of the maintenance provider.
Note hard cutoffs: Periodic filings not submitted by Jan 14 of the following year, or CAT1/CAT5 filings not submitted by the 21st day after the deadline month, are rejected outright and trigger failure-to-file violations.
For defects found on CAT1/periodic inspection, correct within 90 days and file an Affirmation of Correction within 14 days of correction; missing the 104-day mark triggers a Failure-to-Correct violation.
