Steps
CAT1 is an annual no-load safety test performed between Jan 1–Dec 31 each year, filed within 21 days of the test date.
CAT5 is a full-load, full-speed comprehensive test performed every 5 years, timed from the month of the most recent CAT5 test or from the month a Certificate of Compliance was issued for a new elevator.
Both tests must be witnessed by a DOB-approved third-party inspection agency independent of the maintenance contractor, not self-certified by the maintaining company.
File CAT5 results within 21 days of the test date; filings after the 21st day of the month following the five-year anniversary are rejected and trigger a failure-to-file violation.
