Steps
Set up incident categories and site locations first so they populate the report's dropdown fields
Guard fills out the Incident Report form on mobile (or browser with an on-site license) during/after the incident
Tag an incident type and location so the entry feeds Incident Analytics correctly
To change the template later, duplicate it (Settings > Report Templates > Duplicate) rather than editing the live version
Assign the duplicate to target sites and cut over only once in-progress drafts are submitted
Known gotchas
Archiving or deleting fields from a template that already has submitted reports breaks the field-ID mapping and makes historical reports/PDF exports render blank
Leaving the site/location field blank on an incident causes it to show as "N/A" in Incident Category analytics, skewing trend reports
