Steps
Determine whether the system uses a listed, factory-premixed antifreeze solution or a legacy (non-listed) glycerine/propylene glycol solution, since acceptance criteria differ
For listed antifreeze, test annually to confirm the solution is at 100% concentration of the listed product using the manufacturer/listing-specified method
For legacy solutions still permitted in the jurisdiction, measure concentration by refractive index or specific gravity and confirm propylene glycol does not exceed 30% by volume or glycerine 38% by volume (ESFR-listed exceptions aside)
If concentration is out of range or the loop was drained for any reason, fully drain and refill with a listed antifreeze solution rather than topping off
Document antifreeze type, test method, and result at each annual test for the specific loop tested
Known gotchas
NFPA 25 has been phasing out legacy (non-listed) antifreeze acceptance since the 2014-era editions -- many older buildings still have non-listed solutions that are now noncompliant and need conversion, not just testing
Concentration above the code maximum is treated as a deficiency requiring correction -- a stronger-than-spec mix increases combustibility rather than improving freeze protection
Topping off a low antifreeze loop with plain water dilutes concentration below freeze protection without necessarily showing up until winter -- full drain-and-refill is the correct response to any out-of-spec reading
