Steps

  1. Download the current Elevation Certificate, FEMA Form FF-206-FY-22-152, directly from fema.gov/flood-insurance/find-form/underwriting — do not reuse an old cached PDF, since FEMA periodically reissues the form under a new OMB expiration date (this edition is valid through 06/30/2026; check for a newer edition before use).
  2. Complete Section A (property/building identification, including lat/long and at least two to four clear color building photos) and Section B (flood zone, FIRM panel number/effective date, and the source and datum of the Base Flood Elevation).
  3. As the certifying surveyor/engineer, complete Section C with elevations measured to the nearest tenth of a foot, using the same vertical datum cited in Section B, and select the correct building diagram number.
  4. If applicable, use the newer Section H/I add-ons designed to establish first-floor height for NFIP rating purposes without requiring a full elevation certificate survey — confirm with the insurer/AHJ whether these sections are acceptable for the specific transaction.
  5. Sign, seal, and date the certification statement, and retain the surveyed benchmark/vertical control reference used, since a licensed surveyor, engineer, or architect must certify Sections C and G.
