Coverage effective and termination dates in EBV reflect payer enrollment data which may lag real-world enrollment changes by days to weeks; a patient newly enrolled in a plan may not appear in EBV until the payer submits updated eligibility files
EBV covers pharmacy benefit only, not medical benefit; specialty biologics administered in an office or infusion setting bill under the medical benefit and require a separate medical eligibility check via X12 270/271 transactions
