After positive SDOH screening, create a Condition resource conforming to the SDOH Clinical Care Condition profile representing the identified social risk
Construct a Goal resource conforming to the SDOH Clinical Care Goal profile, referencing the Condition as the addresses target
Populate Goal.description with a SNOMED-coded concept from the Gravity-curated ValueSet for the relevant SDOH domain
Set Goal.target with a measureable outcomeCode and a due date
POST both resources as a transaction Bundle and capture returned logical IDs for care plan linkage
Known gotchas
Goal.lifecycleStatus is required; omitting it or using a code outside the bound ValueSet will cause validation failure
The SDOH Condition profile requires at least one category code from the SDOH category ValueSet; a category of 'problem-list-item' alone is insufficient
Referencing the Condition from the Goal requires a literal reference to the persisted Condition; do not use contained resources if the Goal will be independently queried
