Steps

1. Maintain a privilege-level data model in your credentialing system that links each granted clinical privilege to the provider's NPI, the granting facility, privilege category, effective date, and any conditions or restrictions.
2. Align the privilege delineation process with The Joint Commission (TJC) standards: privileges must be competency-based, tied to demonstrated training or experience, and approved by the medical staff.
3. Implement a proctoring workflow for new privileges: track required case counts or evaluations, record proctor attestations, and automatically flag privileges for provisional-to-full conversion when criteria are met.
4. Integrate hospital privileging data with payer credentialing rosters where applicable — facility privileges are a required data element for hospital-based specialists when submitting credentialing files to some payers.
5. Automate privilege expiration alerts: privileges granted for defined terms (e.g., temporary privileges) must be tracked and either renewed or automatically expired at the defined end date.
6. Report adverse privilege actions to the NPDB within 15 days of the action as required by the Healthcare Quality Improvement Act — failure to report is a federal violation.
