Download the full LEIE CSV file from https://oig.hhs.gov/exclusions/leie-database-supplement-downloads/ — the monthly updated file contains all currently excluded individuals and entities.
Load the CSV into a local database table; key fields include LASTNAME, FIRSTNAME, MIDNAME, BUSNAME, NPI, DOB, ADDRESS, EXCLTYPE, EXCLDATE, and REINDATE.
Run a monthly batch job to re-download and replace the full file (OIG archives only the prior 12 months of supplement files; the full file always reflects current exclusions).
Match incoming providers against LEIE using NPI when available; supplement with fuzzy name matching (last name + DOB or last name + address) to catch records without NPI.
Document every screening result with timestamp and file version date for compliance audit purposes; NCQA and URAC standards require documented monthly screening.
Flag any match for human review before taking action — names can collide and NPI mismatches occur; do not auto-exclude based solely on name match.
Known gotchas
OIG has confirmed there is no public API for the LEIE; all automation must be built around the downloadable CSV, not an API endpoint.
Exclusion applies to federal healthcare programs (Medicare, Medicaid); a provider excluded from OIG LEIE cannot be paid by any federal program — employing or contracting with them creates significant liability.
The LEIE does not include SAM.gov debarments; a complete exclusion screening workflow requires checking both OIG LEIE and SAM.gov separately.
