Steps

1. Adopt ISBT 128, the global standard for identification and labeling of blood and other medical products of human origin managed by ICCBBA, rather than a facility-specific labeling scheme
2. Ensure blood product labels carry, at minimum, a unique facility identifier, donation/lot number, product code, and ABO/Rh, consistent with FDA's bar code labeling requirements for blood and blood components
3. Scan ISBT 128 barcodes at both receipt (transfer from blood center to hospital) and bedside administration to positively match patient identity to the product before transfusion, per AABB Standards and Accreditation expectations
4. Confirm scanning hardware and software correctly decode ISBT 128's specific data structures rather than merely reading the underlying linear barcode symbology as generic Code 128
5. If your facility issues its own labels (for example for cellular therapy or tissue products), register your facility identifier and product/nomenclature codes through ICCBBA rather than reusing another facility's identifiers
