Steps
Load a Consent Mode default state before any other tag fires, using an early trigger (e.g. GTM's consent-initialization trigger) or an inline gtag('consent','default', {...}) call at the top of the page
Set all four v2 signals — ad_storage, analytics_storage, ad_user_data, ad_personalization — to 'denied' by default for regions requiring opt-in (e.g. EEA/UK), adjusting regional defaults as appropriate elsewhere
Integrate a Google-certified Consent Management Platform (CMP) to capture the user's actual choice and call gtag('consent','update', {...}) with the four signals once the user responds

Known gotchas
ad_user_data — not ad_storage — is what specifically must be granted for Enhanced Conversions/hashed-PII matching to work; assuming ad_storage alone is sufficient is a common mistake
Consent state set after other tags have already fired doesn't retroactively apply — the default consent call must be the very first thing to execute on page load
Google has continued adjusting how these signals affect linked Ads accounts, so behavior assumed from earlier documentation should be re-verified against current docs rather than treated as fixed
