For each USCDI v4 data class (e.g., Allergies and Intolerances, Goals, Health Insurance Information, Medications), identify the corresponding US Core profile(s) and the specific FHIR elements that represent each USCDI data element
Build a conformance matrix mapping each USCDI v4 data element to the specific US Core profile element, noting the binding strength and value set for coded elements
For new USCDI v4 data classes not present in earlier USCDI versions (e.g., Average Blood Pressure, Encounter Diagnosis), identify the new or updated US Core profiles introduced in 7.0.0 to support them
Validate a sample set of FHIR resources from your system against the US Core 7.0.0 profiles using the FHIR validator to confirm all mapped USCDI v4 elements are correctly represented
Known gotchas
USCDI v4 introduces data elements that may require multiple FHIR resources to represent completely — for example Health Insurance Information maps to both Coverage and Organization resources, requiring both to be present and linked
Not all USCDI v4 data elements have 1-to-1 mappings to single FHIR elements; some data elements map to combinations of coded elements and require specific value set bindings that differ between USCDI versions
ONC certification criteria reference specific USCDI versions — a system certified to USCDI v1 is not automatically compliant with v4 requirements; check the applicable certification criterion before claiming USCDI v4 compliance
