For HUD/VA-financed new construction, use HUD-NPMA-99B 'New Construction Subterranean Termite Service Record' (current form posted at hud.gov)
Confirm the documented elements: treatment type, termiticide product and EPA registration number, dilution rate, application volume, treatment date, and technician/company certification information
Check state building-code overlays - e.g., Florida requires a Certificate of Protective Treatment and a permanent Notice of Termite Protection under Florida Building Code Section R318
Check Texas Structural Pest Control Service pretreatment triggers (wood components within 18 inches of grade, known termite-activity areas, or slab foundations)
Verify the local building department's requirements before certificate of occupancy, since termite pretreat documentation can be a CO prerequisite
Known gotchas
The federal HUD-NPMA-99B form does not preempt state law - state/local documentation requirements can be stricter or use an entirely different form
