Steps

1. For physician and PA license verification, use the FSMB Data Integration service (fsmb.org/data-integration) which provides API and managed file transfer access to primary-source verified licensure and discipline data for over one million physicians and PAs.
2. For nursing license verification, query the Nursys database (nursys.com), which provides primary source equivalent verification directly from state boards of nursing for all US jurisdictions.
3. For states not covered by FSMB or Nursys, automate direct queries to individual state medical board verification portals using their public lookup URLs; store the URL, query method, and field mapping per state in a configuration table.
4. Store verification results with: provider NPI, license number, state, issue date, expiration date, license status, discipline history, and the source/timestamp of the verification.
5. Schedule license status re-checks per NCQA standards: the 2025 NCQA update requires monthly license expiration tracking for credentialing accreditation and certification.
6. Alert credentialing staff when a license is expiring within 60 days, has lapsed, or shows a new disciplinary action — do not auto-terminate without human review.
