Known gotchas
Switch providers do not adjudicate claims—they route them; the switch must have a routing agreement with the target PBM's BIN; if a BIN is not in the switch's routing table, the claim will reject with a routing error rather than a clinical reject
Some PBMs use proprietary edits applied at the switch layer (value-added services); these edits are applied before the claim reaches the PBM and their rejection codes may not be documented in the standard NCPDP reject code set
Switch agreements typically include per-transaction fees; high reversal rates or test transactions in production will increase costs and may trigger compliance review by the switch provider
