In Mangomint, go to Business Setup > Forms and Charting to create a new client form template
Add fields for health history, medical conditions, and design/placement details relevant to tattoo or piercing consent
Mark the form as a required client form and link it to the relevant tattoo/piercing services so it triggers automatically
Enable sending the form before the appointment so clients complete it online without logging into an account
Save completed forms to the client profile for staff to review before the session
Known gotchas
Client forms are a separate form type from staff member forms (SOAP notes/charting) — pick the correct type when building the template
Clients can submit forms without a Mangomint account, but staff must still manually confirm a form was completed before checking in a walk-in
Custom form fields don't include a native minor/guardian-signature toggle — studios serving minors need to build that logic into the form question itself
