Register on the CAQH ProView portal (proview.caqh.org) using your NPI and a valid email address to obtain a CAQH ID.
Complete all required profile sections: personal demographics, education, training, work history, malpractice insurance, hospital privileges, and DEA registration.
Upload supporting documents (DEA certificate, malpractice face sheets, board certifications, state licenses) in accepted formats before attesting.
Authorize participating health plans to access your profile by searching the plan list and granting access within the portal.
Submit the attestation using the digital signature workflow, confirming data accuracy; your profile becomes active immediately after attestation.
Track the 120-day re-attestation cycle (CAQH deactivates profiles after expiration) and set calendar reminders to re-attest before the deadline to avoid payer network drops.
Known gotchas
CAQH requires re-attestation every 120 days; the cycle does not reset until you actively re-attest — missing the window deactivates your profile and can trigger claim denials.
Uploading expired or mismatched documents (e.g., a malpractice face sheet that does not match the declared coverage dates) causes payer verification failures without a clear error message.
CAQH ProView data flows to payers but does NOT replace individual payer credentialing applications — plan-specific enrollment still requires separate submission.
