Set category to an SDOH category code from the SDOH Condition Category value set and code to the appropriate ICD-10 or SNOMED CT code
Reference the patient, the recording practitioner, and, where applicable, the screening QuestionnaireResponse that triggered the finding
Submit or update the Condition resource on the FHIR server and confirm a 201 Created or 200 OK response
