Review the Da Vinci PDex IG's Provenance profile (built on the US Core Provenance profile) to understand required agents, roles, and entity elements for payer-exchanged clinical data.
For each FHIR resource included in a PDex data exchange (clinical resources such as Condition, Observation, MedicationRequest), create a corresponding Provenance resource with Provenance.target referencing the clinical resource.
Populate Provenance.agent with at minimum two agents as required by the PDex/US Core Provenance profile: the author (the original clinical author organization or system) and the transmitter (the payer transmitting the data) — verify required agent roles against the current PDex IG.
Set Provenance.recorded to the timestamp of the data exchange event and Provenance.activity to the appropriate code representing data exchange (verify permissible codes in the PDex IG).
When a payer receives data from another payer and re-shares it, update the Provenance chain by adding the receiving payer as an additional agent or by creating a new Provenance that points to the prior Provenance as an entity.
Include Provenance resources in the PDex data response Bundle so that receiving systems can immediately associate each clinical resource with its provenance without a separate query.
Known gotchas
PDex Provenance has specific agent role codes from a defined value set — using codes outside this set will cause profile validation failures; verify against the current PDex IG.
Provenance resources can become voluminous in large data exchanges — balance completeness with performance by ensuring Provenance is included for every exchanged resource but no more.
The US Core Provenance profile on which PDex builds has evolved across US Core versions; confirm which US Core version the PDex IG references to use the correct profile constraints.
